Need a Waiver

I need a waiver, that i can print and get signed before Sunday. Whatever waiver(s) is required for a Minor to be a passenger. Thanks

The forms are on the SCCA Website, and yes, it took a few minutes for me to find them, not in the obvious place.
Be sure to print the forms in color. If the parent or legal guardian will not be at the event for the registrar to view them
signing the form, then have the form notarized, unless the registrar replies to this posting.

Thank you for the help, is it 3 forms? or just the 2? i am more just confused on which ones are required as a passenger

Just for information, minor must be 12 to ride along and be in hot pits. Feel free to ask us any questions the morning of the event. Minor waiver is one carbon copy page at the event; not sure about the online form. Basically we keep a copy and you get a copy..also put all of 2015 at the top for the date and it will cover the rest of the year. The rider also gets a solid red wristband.
